Astolfo is without doubt Izunia's first friend - though Prompto's arguably closer at the moment, that's just because it's easier to talk because of their shared context.
But by the same token, the lack of shared context makes him more... sure, of Astolfo, in a way? He's something that isn't tied to anything in Eos at all, which is refreshing to be sure, but it's also... Deeply important in a way Izunia hasn't really articulated to himself yet. It's something he did on his own, even if Astolfo's natural friendliness definitely assisted, and that's... meaningful, for someone who is used to living under his brother's shadow in so many ways (some of them deeply fucky).
Also he just makes Izunia have fun a lot of the time, which is also important given that Izunia tends to take things too seriously when left to his own devices. Astolfo is one of the people that brings out his more ridiculous, dramatic side, and just
squishes them THEY'RE SO GOOD

Izunia is really important to Astolfo, undoubtedly. He's really latched onto him with their time spent together, and he really wants to be there for him and help him out with the things he struggles with about himself and his past out of the fact that he just really likes him and wants to see him doing better. Being his first friend, he also hopes that he can help him make other ones too and let more people in and have that love and support he apparently never really knew.
Which is also part of why he loves to be goofy and fun with him too, aside from just being naturally inclined to be that way, getting him to cut loose and seeing him enjoying himself and smile brings him a lot of joy.
He's committed himself to being at Izunia's side now, and with Astolfo being how he is, he'll do that for as long as he can.
